2020-2021 Sports Physical Form

Participation in middle sports is available to all 7th and 8th grade students. 6th graders are welcome to participate in Cross Country and Wrestling.

In the Hybrid model, middle school fall sports will be:

  • “In-House, Intramural” model that focuses on skills and drills
  • Students in Cohort A (last names A - K) attend school on Mondays and Thursdays and will participate with their Cohort group on Mondays and Thursdays
  • Students in Cohort B (last names L - Z) attend school on Tuesdays and Fridays and will participate with their Cohort group on Tuesdays and Fridays
  • Students in the Digital Academy can attend on the days with the Cohort that their last name aligns with
  • 7th and 8th grade students can participate in Boys Soccer, Girls Soccer, Girls Tennis and Cross Country
  • 6th grade students can only participate in Cross Country
  • Because of it being held indoors, we won’t be offering volleyball this fall
  • The season will start on September 21 and end on October 23
  • Students will first need to have a sports physical from a doctor on file in our main office before they can register
  • Physicals are good for 3 years
  • Parents/Guardians will need to register their child on the district’s new activities and athletic system, MyPaymentsPlus, located on the district’s webpage
  • The fee will be $50
  • Registration will be open starting September 8th
  • Practices will start at 3:30 and end at 5:00
  • Child care will be provided for students from the end of their school day at 2:30 until 3:30 when practice begins
  • If you live in the SHMS attendance area, activity buses will be available to bring your child home at 5:00
  • Social distancing and mask wearing guidelines will need to be followed when the athletes are not on the field
  • For the time being, students will be able to change into their athletic clothes in the rest rooms.
